Forms: Request for Surgical Pathology and Non-Gyn Cytology
Collection Instructions:
1. Obtain cervical specimen using a spatula or endocervical
brush.
2. Rinse spatula and endocervical brush in a ThinPrep® vial as
follows:
A. Endocervical brush-swirl 10 times while pressing bristles
against container.
B. Spatula-rinse and swirl 10 times.
3. Discard collection devices.
Note: 1. The following specimens will be
returned to submitting physician:
A. No requisition form
B. Name on requisition form does not match name on specimen
C. Unlabeled specimen
D. No doctor’s name given
2. Label container with patient’s name, date of collection,
physician’s name, and hospital identification number (if
applicable).
3. Specify if Chlamydia
trachomatis/Neisseria gonorrhoeae or
human papilloma Hybrid Capture II® testing is also
requested.
