Forms: Request for Surgical Pathology and Non-Gyn Cytology

Collection Instructions:

1. Routine surgical pathology specimens should be submitted in containers with 10% formalin. (Special kits are available for prostate biopsies.) Cases suspected of immune complex diseases may require an additional specimen in a special fixative.
2. Screw lid securely on specimen container.
3. Label container with patient’s name (first and last) and specimen source.
4. Forward specimen in a biohazard bag.
